How do I get an Illinois birth certificate?

Requirements

• • Eligibility: Only the person named on the certificate, their parents, children, siblings, spouse, or legal representative can order a birth certificate. Other individuals may be eligible to order a birth certificate if they can demonstrate a direct and tangible interest in the record.


• • Required Information: To order a birth certificate, you must provide the full name of the person named on the certificate, their date of birth, place of birth, and the full name(s) of the parent(s) listed on the certificate. You must also provide a valid photo ID (such as a driver's license or passport) and documentation proving your relationship to the person named on the certificate or your eligibility to order the record.

Procedure

You can order a birth certificate online, by mail, or in person at a local vital records office.

• • Online: To order online, visit the Illinois Department of Public Health's Vital Records website and follow the instructions to submit your order and payment.


• • Mail: To order by mail, download and complete the Birth Certificate Request Form from the Vital Records website, and mail it along with a copy of your valid photo ID and payment to the address listed on the form.


• • In Person: To order in person, visit your local vital records office and bring a copy of your valid photo ID, documentation proving your relationship to the person named on the certificate or your eligibility to order the record, and payment for the certificate fee.

Fees

The fee for a certified copy of a birth certificate in Illinois is $15. Additional fees may apply for expedited processing and shipping.

Note: Vital records in Illinois are kept at the county level, so if the birth occurred within the past 75 years, you may need to contact the county clerk's office where the birth occurred to obtain the record.
